only. Removed the checked-in html/latex/rtf output (now gitignored; the
generated HTML is published to lighthub.github.io).
- move references/examples to documentation/core/ and documentation/modules/
- move config samples, images and the doc-v2 changelog to documentation/reference/
- add documentation/README.md as the master index
- remove deprecated AI-generated docs (MIGRATION_GUIDE, START_HERE,
COMPLETION_REPORT, DOCUMENTATION_INDEX, README_v2_0, suffixes_reference v1,
old json config reference, DOCUMENTATION_UPDATE_REPORT)
